MAY 8, 2008   MEDICAL REQUIREMENTS CHANGES

Students entering Kindergarten, Sixth and Ninth grades must submit a physical completed within one year of the first day of this school year. These will be collected at school registration in August. They are due no later than the first day of school attendance. The Fifth Grade physical is no longer required. Children entering Sixth Grade are now required to have a physical prior to the first day of Sixth Grade. Students who had a physical for entrance into Fifth Grade for the 2007-2008 school year do not need to have another one for entry into Sixth Grade this year, unless the child is attending school for the first time.

The Physical must include a Lead screening for Kindergarteners and DIABETES screening for all ages.

In addition, there is a new requirement that all children entering Kindergarten must have a complete eye exam by an optometrist or an ophthalmologist.

Students entering Kindergarten, Second and Sixth grades must have a dental exam prior to May 15 of each school year. For parents’ convenience, Whiteside County will have a mobile dental unit at our school on September 22 to conduct the required dental exams.
